Queensland Government, Australia, Safety Bulletin from August 1999. A 1982 diesel-electric truck burst into flames while undergoing dynamic horsepower testing and adjustment.

Contributing factors to the source of the fire and severity of the incident included:

  • Fuel spray onto hot (turbo) components from a broken fuel line fitting
  • Failure of the suppression system to deploy (due to a discharge event not being rectified)
  • Weak defect management system (not identify fire suppression as a “stop and repair” level threat)
  • Insufficient back-up fire fighting equipment/resources
