Suppression/extinguisher systems not activated, caused by:

  • lack of knowledge of requirements
  • self-preservation (i.e. evacuate before deploying)
  • position of fire suppression manual activation button cannot be accessed (compromised by fire or beyond reach from ground level)
  • reluctance to deploy the suppression system (e.g. due to operator’s perception of the cost of a deployment or unwanted senior management attention)
  • fire extinguisher missing or inoperable
  • awkward access to or excessively heavy on-board portable fire extinguisher(s)

 Credible Failure mode is addressed by:

  • BI-01.01 Trained personnel who are supported by an appropriate and up-to-date training management system
  • BI-01.04 Trained, Competent and Authorised Mobile Equipment Operators
  • BI-01.39 Alarm and vehicle information panel expected response – information prepared for operators
  • BI-09.01 Emergency Preparedness
  • BI-09B.02 General Fire Training
  • BI-09B.03 Operator Fire Training
