Failure of the reticulated fire suppression system to work, caused by:
- battery back-up fails when vehicle is isolated (affecting nylon detector tube, linear detector wire or heat probes)
- Fire system activation not effectively actuated e.g. operator does not press long enough or hard enough
- Inadequate pre-start inspection checks of FSS
Credible Failure mode is addressed by:
