Suppression/extinguisher systems not activated, caused by:
- lack of knowledge of requirements
- self-preservation (i.e. evacuate before deploying)
- position of fire suppression manual activation button cannot be accessed (compromised by fire or beyond reach from ground level)
- reluctance to deploy the suppression system (e.g. due to operator’s perception of the cost of a deployment or unwanted senior management attention)
- fire extinguisher missing or inoperable
- awkward access to or excessively heavy on-board portable fire extinguisher(s)
Credible Failure mode is addressed by:
- BI-01.01 Trained personnel who are supported by an appropriate and up-to-date training management system
- BI-01.04 Trained, Competent and Authorised Mobile Equipment Operators
- BI-01.39 Alarm and vehicle information panel expected response – information prepared for operators
