This group of failure modes relates to errors made during the planning and designs of the road network.  It includes:

  • Errors and inadequacies in the mine traffic management plan
  • Not considering vehicle paths and interactions during the design process
  • Failing to allow for operating grades and curves, leading to more difficult speed control for operators
  • Not considering segregation for vehicles of different size or type operating on the site road network
  • Having loading requirements not matched to the capacity of site vehicles
