This group of failure modes arise as a result of the designs which impact mobile equipment interactions.  It includes:

  • Lack of, or inadequate, access controls (physical segregation and signage)
  • Failure to consider vehicle limitations in the establishment of designs 
  • Developing communication protocols which are confusing or difficult to comply with/follow
  • Layouts which limit sightlines (particularly with moving vehicles approaching an intersection)
  • Failing to allow for an unhindered response in the event of an emergency
