Tyre handler operator is unaware of the presence of pedestrian in an operational area resulting in a mobile equipment pedestrian interaction caused by:

  • Tyre or wheel assembly obscures tyre handler operator vision, and
  • Pedestrian not on a defined walking path e.g. takes a short cut
  • Authorised pedestrian working in an operational area not detected by mobile equipment operator.

Credible Failure mode is addressed by:

  • BI.01.10 Tyre hazard awareness for maintainers
  • BI.03.02 Protocols for all aspects of wheel assembly maintenance and tyre repair
  • BI.03.08 System in place for managing interfaces between tyre handler operator and tyre service personnel
  • BI.03.10 Safe and productive tyre handler operation
