Equipment is operated outside equipment design limits:
- Loosening components allowing them to move and contact hot surfaces
- Insulation and heat deflecting components are lost during operations allowing previously protected components to become a fuel source through exposure to hot surfaces
Credible Failure mode is addressed by:
- BI-02.31 Operator prestart checks include brakes, steering, tyres and running gear
- BI-03.02 Operators follow site parking requirements
- BI-04.02 Appropriate system for scheduled inspections including structured checklists and defect reporting
