Queensland Government, Australia, Safety Bulletin from February 2004. The rear hub of a dual set of wheels on a rear dump truck cracked around the circumference and parted.
Detailed analysis highlighted the movement which occurred after the rim damage which cut through a brake line and initiated a fire that ultimately destroyed the entire truck. Recommendations are made to:
- NDT truck wheels and rims on a regular basis
- Alert maintainers to the potential losses which can arise from cracked rims
- Adopt leading practice tyre/wheel maintenance processes
