This group of failure modes relate to mobile equipment being compromised due to errors made in design or manufacturing. The listing in detail is shown in the Important Documents section on the right – but in essence, they relate to:
- Designs that don’t consider load cases on liquid fuel storages and lines
- Use of flammable components where non-flammable equivalents were available
- Not communicating maintenance requirements to end users
- Failing to require fire detection lines to be well protected
- Poor implementation of maintenance strategies at a site, and
- Not including parameters from regulators in the designs.
