This group of failure modes relates to operator errors created or made worse by business systems present.  The types of failure mode identified include:

  • Poor training or supervisor reinforcement of important rules/equipment features;
  • Unfairness in performance management systems;
  • Regulator reactions to events and conditions at the site.
