This group of failure modes relates to the general system controls of short term/operational planning and/or issuing of instructions by supervisors etc.  It includes:

  • Not setting up for success in terms of arrangements for work rosters and monitoring of fatigue levels
  • Not setting up monitoring (unit key system alarms, speed monitoring, etc.) of mobile plant in a consolidated fashion or not reacting to non-conformances in real-time
  • Poor management of change
  • Inconsistent instructions to operational or maintenance workers
  • Failure to adequately monitor worker actions and/or inconsistent application of performance management (discipline) requirements
  • Not monitoring and/or reacting to poor standards of maintenance
  • Failing to allow sufficient resources to achieve an adequate Emergency Response
